Membership Application (MS Word -- updated in Aug. 08)

BYLAWS

To Apply for AAES Membership:

1.Read the current AAES Bylaws on this website.

2. If you meet the bylaws criteria and are interested in becoming a member of the AAES, you will need to identify and contact a Sponsor who is an Active or Senior AAES member.  * Note that it is no longer necessary for the Sponsor to personally mail in or sign your completed application.

3. Download the membership application from the MS Word file above.  Complete the form using a typewriter or a computer (not in longhand).  Print, sign, and mail (do not email) the application form to the AAES Secretary-Treasurer by January 31, 2009, together with these obligatory and necessary documents:

a. Your bibliography/curriculum vitae

b. A reprint of your best contribution to the endocrine surgery literature

c. A list of your endocrine surgical procedures performed during the prior 24 months.  This list must comply with patient privacy laws.

d. As specified in the bylaws, applications for the Active and Allied Specialist membership categories must provide the date of attendance at a prior AAES meeting.  Planning to attend an upcoming meeting does not meet this qualification.  Applicants for AAES Corresponding membership are excused from the prior meeting requirement.

4. These documents must also be received by January 31, 2009 by the Secretary-Treasurer:

a. A detailed letter from the Sponsor addressed to the Secretary-Treasurer.

b. Two letters from additional Endorsers, who must be AAES members (of any membership category.)

Incomplete or late applications will not be reviewed this cycle. Complete applications received by January 31, 2009 will be reviewed by the Membership Committee and by AAES Council, and the names of applicants recommended for election to membership will be voted upon at the 2009 annual AAES business meeting in Madison WI.  Applicants thusly elected to membership will be presented with certificates of AAES membership at the 2010 AAES meeting in Pittsburgh PA.

5. Completed applications received by the Secretary-Treasurer on or before January 31, 2009 will be reviewed by the Membership Committee and AAES Council. Names of prospective members recommended by Council for election to membership will be voted upon by AAES members at the May 2009 Business Meeting. Applicants thusly elected to membership in 2009 will be presented with certificates of membership at the 2010 AAES Annual meeting.

6. Please mail completed applications by January 31, 2009 to:
